Under the supervision of the program manager, direct care providers are responsible for providing instruction and support to clients in a fashion that promotes independence and self-advocacy. Services to participants are delivered based on person-centered support plans and health care plans. Promoting client choice, dignity, respect, and confidentiality are priorities.
Shift: First
Schedule: Monday Through Friday 7:30 am to 3:30 pm
Responsibilities
- Advance independence through instruction and support in areas selected by the clients as identified in their person-centered support plan.
- Participate and advocate for clients in person-centered support plan meetings, basis assessments and/or team meetings.
- Train, assist, and support clients to be self-advocates.
- Chart progress on required documentation.
- Prepare and organize materials and supplies needed to implement programming activities.
- Assist the activity coordinator in developing quality monthly activity calendars.
- Assist clients, as needed, with personal care and hygiene needs through individual attention and assistance in a dignified/sensitive manner.
- Direct Care Providers will assist with maintaining a clean, clutter-free, aesthetic, and safe environment suitable for clients.
- Assess and document blood pressure and weights on clients as required by regulations. Glucose monitoring and documentation as needed per physician orders.
- All staff will practice universal precautions.
- Maintaining program/agency inventory by reporting needed supplies.
- Maintain the health and welfare of clients served.
- Direct Care Providers will communicate with their direct supervisor regarding any concerns and/or safety issues related to clients, staff and/or facility.
- Direct Care Providers will maintain professional and respectful working relationships both within the agency and community.
- Direct Care Providers will represent the agency positively and professionally in accordance with Catholic Charities “Best Practice Tenets.”
- Direct Care Providers will adhere to designated working hours unless prior arrangements were made according to policy.
- Ensure clients are secured for transport according to PCSP.
- Ability to obtain (within first six months of employment) and maintain a CDL.
- Performs other related duties as assigned.
CNA will perform the duties above and in addition:
- Know regulations that apply to Aging Licensure only.
CMA will perform the duties above and in addition:
- Dispense medications and treatments as directed by the health service manager and/or physician.
Requirements
- Education
- High School diploma or equivalent required.
- Prefer 1-year I/DD experience Certified Nursing Assistant (CNA) or Certified Medication Aid (CMA) may be required for some positions.
- Certificate, Registration or License
- Valid driver's license, motor vehicle, and insurance required.
- Commercial Driver's License within the first six months of employment.
- Experience
- One year experience working with the geriatric population and/or individuals with disabilities preferred.
- Skills and Abilities
- Ability to use a personal computer via mouse and keyboard, familiarity with Microsoft Windows products.
- Knowledge and ability to obtain blood sugars via finger stick.
- Knowledge and ability to obtain vital signs.
- Communication
- The ability to articulate well with clients, families, and co-workers is imperative to provide or discover information, teach, and help provide clarity.
- Exposures
- Exposure to human bodily fluids is possible.
- Exposure to climate changes due to outside activities and driving of routes or outings.
Physical Demands
- Work Type: Moderately physical, exerting up to 100 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or negligible amount of force constantly to move objects.
- Climbing/Balancing/Freedom to Move: Controlled mobility in all aspects of ambulation is needed frequently.
- Stooping/Bending: The ability to flex (bend), extend (straightening), and rotate the body is done frequently.
- Reaching: Occasional overhead as well as horizontal and downward movement
- Vision: Adequate to perform job responsibilities
- Color Vision: No particular color vision requirements
- Hearing: Adequate to perform job responsibilities
- Speech: Adequate to express ideas using spoken word and perform job responsibilities.
- Eye/Hand/Foot/Coordination: Frequently operates equipment or performs duties requiring moderate ability.
- Manual Dexterity: Frequently performs duties requiring moderate ability.
